Primary Responsibilities:

Complete annual home health risk assessments for Medicare Advantage health plan members Evaluate prior and current medical history, health conditions, and medication history Educate patients on their conditions and medication regimens Identify care gaps and recommend appropriate follow-up actions Focus exclusively on assessmentsno treatment planning or prescribing required During your shifts, you will visit homes in Peoria and neighboring communities such as Chillicothe and Bartonville. Each risk assessment typically requires about 45 minutes and may include an annual physical or post-discharge evaluation.

Qualifications:

Active Nurse Practitioner license in Illinois Current CPR certification (BLS) Board Certified through AANP or ANCC Completion of an MSN degree from an accredited nursing program Minimum six months of recent NP experience preferred In this role, you'll be compensated $80$90 for each completed evaluation, with additional mileage reimbursement and payment for administrative time.
