Position Summary As a Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes while working independently with the support of an experienced clinical leadership team. You'll complete comprehensive assessments, coordinate care, educate patients and families, and play a vital role in helping patients achieve their healthcare goals. Responsibilities Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and routine skilled nursing visits. Perform comprehensive nursing assessments. Develop, implement, and update individualized plans of care. Administer medications, wound care, injections, and other skilled nursing treatments. Educate patients and caregivers on disease management, medications, and home safety. Collaborate with physicians, therapists, and the interdisciplinary team to coordinate patient care. Complete documentation in Axxess EMR utilizing AI-assisted Start of Care (SOC) documentation. Why Join Astra Health? Part-Time:

Routine Visit:
1 point Qualifications Active Illinois Registered Nurse (RN) license Minimum two years of Med-Surg nursing experience Home Health experience preferred, but not required Valid driver's license and reliable transportation Strong assessment, communication, and documentation skills Service Area This position serves patients throughout Ottawa and LaSalle County.
