Key Responsibilities:

Perform detailed home health risk assessments for Medicare Advantage members Review and evaluate members medical history, health conditions, and medication lists Educate members about their healthcare conditions and medications Identify gaps in care, offering actionable recommendations No requirements to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Illinois Current BLS CPR certification Board Certification (AANP or ANCC) MSN degree from an accredited nursing school At least six months of recent Nurse Practitioner experience preferred
