Job Description

MDS Nurse THRYVE of South Holland is seeking a skilled and detail-oriented MDS Nurse to join our clinical team. The MDS Nurse is responsible for completing accurate and timely resident assessments, coordinating care planning processes, and ensuring compliance with CMS, state, and federal regulations. This role works closely with the interdisciplinary team to support quality outcomes, accurate documentation, and exceptional resident care. Job Summary The MDS Nurse plays a critical role in the assessment and care planning process by completing MDS 3.0 assessments, reviewing clinical documentation, collaborating with interdisciplinary team members, and ensuring resident information is accurately captured and maintained. The ideal candidate has strong knowledge of long-term care regulations, assessment processes, and resident-centered care planning. Responsibilities Complete accurate and timely Minimum Data Set (MDS) 3.0 assessments in accordance with CMS and state regulations. Review resident medical records and clinical documentation to ensure accuracy and completeness. Coordinate the assessment process with Nursing, Therapy, Social Services, Dietary, and other interdisciplinary team members. Participate in care plan meetings and contribute to individualized resident care planning. Monitor changes in resident condition and ensure appropriate documentation updates. Ensure MDS assessments and related documentation support quality measures and reimbursement requirements. Assist with maintaining compliance with the RAI Manual, CMS guidelines, and regulatory requirements. Identify documentation opportunities and collaborate with clinical staff to improve accuracy and quality outcomes. Maintain accurate records while protecting resident confidentiality in accordance with HIPAA regulations. Utilize electronic medical records (EMR) systems to complete assessments and maintain resident information. Perform additional duties as assigned. Qualifications Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required. Previous MDS experience in a skilled nursing facility or long-term care setting preferred. Strong knowledge of
MDS 3.0, RAI
process, care planning, and CMS regulations. Experience with PointClickCare or other electronic medical record systems preferred. Strong clinical assessment, organizational, and critical-thinking skills. Ability to manage deadlines and prioritize multiple responsibilities. Excellent communication skills and ability to collaborate with an interdisciplinary team. Detail-oriented with a commitment to accuracy and quality resident care.
