Job Description

LifeBridge Health, Inc. seeks a Registered Nurse for our acute care team in Baltimore, MD. In this role, you will deliver evidence-based, patient-centered nursing care across diverse adult populations. Responsibilities include comprehensive assessment, IV therapy and medication administration, care planning, monitoring patient status, implementing provider orders, and maintaining accurate EHR documentation. You will educate patients and families, support safe transitions of care, and collaborate with an interdisciplinary team in a compassionate, mission-driven environment that values professional growth and innovation. Responsibilities Provide comprehensive nursing assessment and ongoing monitoring of acute care patients Administer medications, IV therapies, and treatments safely and accurately Develop, implement, and evaluate individualized care plans Document all care, assessments, and interventions in the EHR in a timely manner Collaborate with physicians, therapists, and other team members to coordinate care Educate patients and families on diagnoses, medications, and discharge instructions Follow infection prevention, patient safety, and quality standards and protocols Respond to changes in patient condition and escalate appropriately Support safe admissions, transfers, and discharges within the hospital Participate in continuing education, competency validation, and quality improvement initiatives Required Skills Patient assessment Acute care nursing IV therapy and medication administration Care planning and coordination Electronic Health Records (EHR) documentation Wound care and dressing changes Patient and family education Vital signs monitoring and interpretation Infection control practices Interdisciplinary team collaboration
