Assess patients and respond to changing clinical needs. Deliver symptom management and comfort-focused nursing interventions. Educate patients, caregivers, and families while providing guidance and reassurance. Coordinate care with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team. Provide care in private homes, assisted living communities, memory care, and skilled nursing facilities. Complete timely and accurate clinical documentation. Help patients remain comfortable at home while reducing unnecessary hospitalizations. What We're Looking For Active Minnesota Registered Nurse (RN) license. Hospice experience is preferred but not required. Nurses with experience in ICU, ER, Oncology, Critical Care, Palliative Care, Home Health, Med Surg, or hospital-based nursing are encouraged to apply. Strong assessment skills and sound clinical judgment. Ability to work independently and confidently in the field. Excellent communication and compassionate patient care skills.
