Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.

In this position, the Home Health RN provides skilled nursing care to adult and geriatric patients in their homes, focusing on recovery, chronic disease management, safety, and patient independence. The nurse conducts comprehensive assessments, develops and updates individualized care plans, and collaborates closely with physicians, therapists, home health aides, and family caregivers. Responsibilities commonly include wound care, medication management and education, disease process teaching, monitoring of vital signs and symptoms, and proactive interventions to help prevent rehospitalization. A typical day includes traveling between patient homes within an assigned geographic area around Hillsboro and Jefferson County, performing scheduled visits, and documenting care in an EMR system. The role requires comfort working independently in varied home environments while maintaining strong communication with the clinical team. Visit volume and caseload are structured to support thorough, patient-centered care, allowing time to build meaningful relationships and deliver personalized education. Shifts typically follow a daytime schedule with some flexibility based on patient needs, including occasional on-call or weekend rotations as required.
