Home health, Oasis, HCHB (Homecare Homebase) Responsibilities Conduct comprehensive assessments of high-risk members and develop individualized, cost-effective care plans with providers. Coordinate care transitions and collaborate with physicians, social workers, discharge planners, and other healthcare professionals. Arrange non-medical support such as housing and transportation to improve treatment-plan compliance and outcomes. Monitor clinical, functional, and financial outcomes while identifying opportunities for health promotion, prevention, and early intervention. Advocate for patients by protecting privacy and confidentiality and ensuring appropriate, high-quality care. #
