Oasis (required), HCHB (required), Home health and Home care experience Responsibilities Assess patient health needs, implement care plans, and provide health maintenance, disease prevention, and case management. Coordinate care with physicians and multidisciplinary teams to address patient health and safety needs. Manage medications, physician orders, appointments, hospital transitions, and required nursing documentation. Supervise LPN/LVN staff and ensure proper training of nursing and unlicensed staff. Maintain compliance with health, safety, infection prevention, medication, and patient-care policies. #
