We are seeking skilled professionals to provide home health risk assessments for Medicare Advantage plan members throughout Darlington County, SCincluding the communities of Darlington, Hartsville, Lamar, and surrounding areas.

Primary Responsibilities:

Conduct comprehensive home health risk assessments, typically lasting about 45 minutes per visit, for Medicare Advantage members Review each patients prior and current medical history, medical conditions, and medication lists Offer education to patients regarding their health conditions and prescribed medications Identify potential gaps in required care and recommend solutions to address these gaps

This role does not require the creation of treatment plans or the prescribing of medications

Required Qualifications:

Active and unrestricted Nurse Practitioner license in South Carolina Current CPR certification (BLS) Board Certification as a Nurse Practitioner through AANP or ANCC Minimum of 6 months current NP experience preferred Master of Science in Nursing (MSN) from an accredited institution Nurse Practitioners in this role receive $80$90 per evaluation, plus a mileage stipend and administrative time compensation.
