Key Responsibilities:

Perform thorough home health risk assessments for Medicare Advantage plan members Review and evaluate patients medical and medication histories, including both prior and current conditions Educate patients and families on diagnosis, medical conditions, and medication management Identify any gaps in care and provide recommendations for follow-up and resolution No requirements to create treatment plans or prescribe medications

Qualifications:

Current Nurse Practitioner license in South Carolina Active BLS (CPR) Certification Board Certification through AANP or ANCC Minimum six months of Nurse Practitioner experience preferred MSN degree from an accredited nursing program
