Key Responsibilities:

Conduct 45-minute annual home health risk assessments and post-discharge visits in patients homes Review and evaluate medical history, current health conditions, and medication usage Deliver patient education on health conditions and medication management Identify gaps in care, and suggest evidence-based recommendations for care improvement

Note:

No involvement in medication prescribing or treatment plan development; focus is on assessment and education

Qualifications:

Active Nurse Practitioner license in South Carolina Board certification through AANP or ANCC Current CPR certification (BLS) Graduate of an accredited MSN program Minimum six months of recent NP experience preferred This role offers competitive compensation of $80-$90 per evaluation, along with a mileage stipend and paid administrative time.
