Trenton Therapy LLC is seeking a PRN Physical Therapist (PT) or Physical Therapist Assistant (PTA) to provide home health therapy visits in patients' homes. This is a flexible, per-visit PRN role for a clinician who is dependable, organized, and comfortable working independently in the field while communicating clearly with the home health team. In this role, you will complete scheduled in-home visits, provide skilled therapy interventions focused on functional improvement and safety, educate patients and caregivers, and complete documentation accurately and on time per home health requirements. The PT will perform evaluations, establish and update the plan of care, and complete reassessments and discharges as required. The PTA will treat under the supervising PT's plan of care, document patient response and progress, and promptly communicate changes, concerns, and barriers to the supervising PT. This position requires an active PT or PTA license in Texas, reliable transportation, a valid driver's license and auto insurance, and strong communication and time-management skills. Home health experience is preferred but not required for the right clinician.

Pay:
