Job Description

Intake Support Specialist Ready to be a source of hope and support in your community? The Homeless Prevention Center (HPC) seeks a motivated individual to support our Coordinated Entry process, helping clients and prospective clients access assessments and services. This is an in-person, office-based position serving Rutland County, Vermont. It does not require previous case management experience, home visits, or transporting clients in a personal vehicle. The Specialist will be hired by the Executive Director and supervised by the Administrative Assistant.
Principal and Administrative Duties:
Answering Multi-line Phone System and Directing Calls to appropriate staff Greeting Clients & Other Agency Professionals in our lobby Completing Screening forms to help determine eligibility for services Supporting Coordinated Entry process with filing and data entry Contacting clients for appointment confirmations and reminder calls Rescheduling appointments for clients and documenting in case notes Creating, scanning and saving documents to electronic client files Assisting with mailings Acting as first point of contact for individuals and families seeking assistance Explaining HPC's purpose and programs to guests Documenting information dispensed and/or referrals made in case notes Being empathetic, professional and balanced when dealing with people in difficult or emotional situations Assisting co-workers and Management as needed
Qualifications:
Computer skills required, including Microsoft Office suite (Outlook, Excel, Word). Pleasant demeanor, dependability and appropriate office attire are essential. The Specialist should be able to exercise flexibility in approaches while maintaining professional integrity and organizational values; be detail-oriented and consistent with keeping accurate records; be willing to communicate with people who have mental health or substance abuse challenges. Some knowledge of local community resources is a plus. This position expects a person to be non-judgmental, compassionate, and possess an ability to communicate and work effectively with people from all walks of life.
Terms of Employment:
The Intake Specialist is a full-time position with benefits offered by HPC, including paid vacation and paid personal time, paid sick time, paid holidays and a voluntary
SIMPLE IRA
plan. Eligible for an employer-based health plan after six months. The Specialist must have access to reliable transportation. This individual will be subject to an initial probationary period.
