Job Description

Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) - Staff Nurse, where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate nursing services across various clinical settings, ensuring high-quality care tailored to each individual's needs. Responsibilities Deliver comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to inform personalized care plans. Administer medications and injections accurately following established protocols, including IV infusions and medication management using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Epic, Cerner, or Athenahealth. Assist with complex procedures such as catheterization, wound suturing, tube feeding, and sterile processing techniques while maintaining aseptic technique standards. Monitor patients' physiological responses during treatments like ventilator management, cardiac catheterization, or spinal taps; respond swiftly to acute pain or emergent situations in emergency room or ICU settings. Collaborate with multidisciplinary teams in settings like long-term care facilities, assisted living communities, hospitals—including Level I Trauma Centers—and outpatient clinics to coordinate discharge planning and case management efforts. Maintain meticulous medical documentation adhering to HIPAA regulations; ensure accurate
ICD-9/ICD-10
coding for diagnoses and procedures. Support infection control protocols and participate in patient education on topics such as dementia care, Alzheimer's disease, or chronic disease management to promote health coaching and preventative care strategies. Skills Proven experience with Epic EMR systems and proficiency in medical documentation practices. Strong background in critical care environments such as ICU, emergency medicine, or trauma centers; familiarity with Level II Trauma Center standards preferred. Extensive knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to diverse patient populations including pediatrics, geriatrics, neonatal ventilator management, and behavioral health. Skilled in performing venipuncture, phlebotomy, injection administration, catheter care, and specimen collection/processing with aseptic technique. Ability to work effectively with individuals with disabilities or developmental delays; experience in dementia or memory care enhances patient support capabilities. Competent in triage procedures within urgent care or outpatient settings; capable of rapid assessment and prioritization of patient needs. Excellent communication skills for collaborating with healthcare teams across hospital medicine, home & community care environments, and post-acute settings. Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures like dermal fillers or botulinum toxin treatments; familiarity with medical imaging and diagnostic evaluation processes is advantageous.
