Triple Ace Home Health Agency is seeking a motivated and professional Home Health Community Outreach Specialist to promote the agency's home health services throughout the community. The Outreach Specialist will establish and maintain relationships with physicians, hospitals, rehabilitation centers, skilled nursing facilities, assisted living communities, case managers, social workers, and other healthcare professionals to increase awareness of the agency and support lawful referral development. This position requires excellent communication, organizational skills, and knowledge of the healthcare industry. The ideal candidate is self-motivated, dependable, and passionate about serving patients and the community. Essential Duties and Responsibilities Develop and maintain relationships with physicians, hospitals, clinics, discharge planners, case managers, rehabilitation facilities, and community organizations. Promote the agency's skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work, and home health aide services. Conduct in-person visits to referral sources within the assigned territory. Attend community events, health fairs, networking meetings, and educational programs to promote agency services. Deliver marketing materials and educate referral sources regarding admission criteria and available services. Follow up on referral opportunities and maintain ongoing communication with referral partners. Maintain accurate records of outreach activities and submit weekly activity reports. Collaborate with agency leadership to develop and implement marketing strategies. Identify opportunities for business growth while maintaining high ethical standards. Represent Triple Ace Home Health Agency in a professional manner at all times. Maintain confidentiality and comply with HIPAA requirements. Perform other duties as assigned. Qualifications High school diploma or GED required; Associate's or Bachelor's degree preferred. Previous experience in healthcare marketing, community outreach, physician liaison, pharmaceutical sales, or home health marketing preferred. Excellent interpersonal, communication, and presentation skills. Ability to work independently and manage multiple priorities. Basic computer proficiency, including Microsoft Office and email. Valid driver's license, reliable transportation, and current automobile insurance. Ability to travel throughout the assigned service area. Knowledge and Skills Strong networking and relationship-building abilities. Professional appearance and excellent customer service skills. Excellent verbal and written communication. Strong organizational and time management skills. Ability to maintain confidentiality. Understanding of healthcare regulations and ethical marketing practices. Work Schedule Part-time (approximately 15-25 hours per week or as assigned). Some evening or weekend community events may be required. Physical Requirements Frequent driving within the service area. Ability to lift and carry up to 25 pounds of marketing materials. Ability to sit, stand, walk, and use a computer for extended periods. Employees are prohibited from offering or accepting improper inducements for patient referrals. Employment Status This is a part-time, at-will position. Employment may be terminated by either the employee or Triple Ace Home Health Agency at any time, with or without cause or notice, subject to applicable law.
