Overview Join our dynamic healthcare team as a Pharmacist, where you will play a vital role in delivering exceptional patient care and ensuring the safe, effective use of medications. Responsibilities Dispense prescriptions accurately while verifying dosage, interactions, and contraindications using advanced pharmacy software and electronic health record (EHR) systems Continuous conversations with top clinical providers in the area to help manage the patient's medications Conduct thorough patient assessments to determine appropriate medication therapy plans, including immunizations and medication administration protocols Provide expert consultation on complex medication regimens, including IV infusions, compounding medications, and pain management strategies Collaborate with medical teams to develop and implement comprehensive medical management plans tailored to individual patient needs Manage inventory levels efficiently, ensuring proper storage, aseptic technique during compounding, and compliance with HIPAA regulations Perform patient care activities such as medication reconciliation, patient education on physiology and anatomy relevant to their treatment, and supporting pediatrics-specific therapies Maintain accurate documentation of all pharmacy activities within EMR (Electronic Medical Records) and EHR systems while adhering to confidentiality standards Requirements Valid pharmacist license with current registration in the applicable jurisdiction Proven experience in LTC (Long-Term Care) pharmacy or hospital settings with a focus on acute care environments Critical care experience preferred; familiarity with hospital workflows and medical terminology essential Clinical knowledge is a must Demonstrated clinical pharmacy experience including patient service, assessment skills, and medical management expertise Proficiency with pharmacy software platforms, EMR/EHR systems, inventory management tools, and IV insertion techniques Knowledge of physiology, anatomy, immunizations protocols, aseptic techniques for compounding medications and IV infusions Ability to perform medication administration accurately while maintaining high standards of patient safety and HIPAA compliance Strong communication skills complemented by pharmacy technician experience that supports seamless workflow coordination Embark on a rewarding career where your expertise makes a real difference—apply today to join our committed team dedicated to advancing healthcare excellence!
