Sign on Bonus Merit Increases Referral Bonuses End of Year Bonuses Caseload Bonuses Average salary with certifications, year-end bonuses, caseload bonuses, mileage, etc. $85,000-90,000 The RN (Registered Nurse) Case Manager provides case management and nursing services to Hudson Valley Hospice patients and their families and to ensure that those services are consistent with the philosophy, goals, objectives and policies of the Agency. Register Nurse (RN) Case Managers work in various settings: currently looking for nursing home/facility setting. Each RN Case Manager provides pain and symptom management, recommendations and collaborates with the other Interdisciplinary Team members. We are looking for RNs to join our home care and nursing home teams!
What you will do:
Perform initial and ongoing patient/family, physical, emotional, psycho-social, cultural and spiritual assessments to ensure and coordinate the delivery of comprehensive, services by the Interdisciplinary Hospice Team. Provide palliative nursing care and symptom control in accordance with physician orders and plan of care. Documents and enters into EMR all interventions of phone and home visits Provide anticipatory guidance to the patient/family to assist in preparing for coping with events as they occur. Teach, supervise and counsel patients/families/care givers regarding care of patient. Communicates patient status to family members and/or caregivers.Skills we are looking for:
Ability to work non-judgmentally with patient/family of any culture, religion, socio-economic background or life-style. Ability to effectively communicate verbally (in person and by phone) and in writing. Work cooperatively independently and as a team member. Prioritize and handle multiple tasks Organize work with efficient time management skillsPreferred Education & Experience:
Graduate of an accredited nursing school. Valid New York State nursing license. Two years nursing experience preferred One year medical-surgery experience preferredWork Environment:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Work in various home environments, nursing homes or hospitals that Hudson Valley Hospice is contracted with Moderate/High level of possible exposure to infectious agents. Moderate amount of travel by car Moderate to high risk of exposure to unsafe environments. Routine duties involve potential for direct exposure to blood or body fluids. Valid New York State drivers license with reliable transportation.Hours:
Monday-Friday (8:30am-4:30pm); on call rotation (1-3 times per year)Pay:
