Job Description
Valley Vista Care Center of St. Maries located in beautiful St. Maries Idaho is currently seeking a full-time Resident Services Coordinator to join our team! SUMMARY :
Counsels and aids residents and families by performing the following duties.ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following.Other duties may be assigned:
Prepares ABN and NOMNC for the facility, and submits to the state monthly. Conducts preadmission and admission meetings with families and prospective residents when appropriate or requested. Coordinates all admissions and discharges. Refers and assist residents to obtain community resources available to them as part of their care plan and/or discharge arrangements. Completes resident's social history report. Advocates for resident rights and compliance with OBRA regulations according to Federal Regulations. Coordinates services with other disciplines in the facility to enable residents and families to promote their own well-being. Completes assessment of residents and development of the Trauma Informed Care on new residents. Communicates with Ombudsman as needed. Investigate, resolve and report grievances made by residents, family and visitors. Develops a discharge plan for all residents with appropriate plan to best meet their needs. Maintains and utilizes a comprehensive community resource file. Provides crisis intervention for residents and their families when appropriate. Documents Resident Service interventions, involvement in the progress notes in the resident medical record. Participates in resident care planning. Coordinates room and/or roommate change with the Nursing Department, and notifies all parties involved along with documenting notification in the resident's medical record. Assures compliance with federal guidelines pertaining to the Resident Services Department. Complies with all federal and state regulatory guidelines regarding residents. Ensures a home-like environment for each resident. Works directly with Activities Department to ensure each resident receives as many activities as possible. Obtains and conducts the residents shopping requests on a weekly basis, and distribute accordingly, Updates resident demographics and name plates. Works within allowed RSC budget. Schedules and participates in Care Conferences. Completes and maintains inquiry log. Maintains an up-to-date resident roster. Prepares and maintains emergency lists and necessary paperwork. Distributes financial mail to residents. Assists with obtaining POA's and completing Medicaid applications. Prepares and distributes communications through the designated platform. Assists residents with their personal belongings. Assists in maintaining resident trust and spend down needs. Oversees petty cash for resident trust. Submits the Admit/Discharge Report monthly to the Ombudsman. Coordinates family visitations, and schedules accordingly. Serves as a backup to the Complex Care Resident Services Coordinator.QUALIFICATIONS
