Apply in-person at 405 kings way, Suisun City 94585 Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote dignity and independence. Monitor patients' health by observing vital signs, reporting changes, and maintaining accurate care plans aligned with medical directives. Support medication administration following HIPAA regulations and prescribed protocols to ensure safety and compliance. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Facilitate patient transfer methods using equipment such as Hoyer lifts or patient transfer techniques to ensure safe mobility. Maintain a clean and safe environment by performing regular cleaning tasks and adhering to infection control standards. Provide emotional support through active listening, engaging conversation, and companionship to foster trust and well-being. Qualifications Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care settings such as assisted living and memory care. Knowledge of patient observation techniques, patient monitoring, and dementia or Alzheimer's care practices. Ability to perform manual handling tasks including heavy lifting and patient transfers safely. Certification in CPR and First Aid is required; additional certifications like HIPAA training are preferred. Familiarity with care plans, medication administration procedures, and resident's rights regulations. Strong communication skills for effective client interaction and team collaboration. Prior experience with home & community care environments or hospice care is highly desirable. Must be able to pass back ground clearance. AM,PM and NOC shifts are available.
