Responsibilities Provide direct personal care assistance, including help with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods like Hoyer lifts and manual handling techniques Monitor patients' vital signs and observe for any changes in health status or behavior, documenting findings accurately while adhering to HIPAA regulations for patient privacy Assist with medication administration following prescribed care plans and ensure proper documentation of all medication-related activities Support residents with meal preparation, ensuring nutritional needs are met while accommodating dietary restrictions or preferences Facilitate patient transfer methods safely during movement between beds, chairs, or transportation using proper patient transfer techniques Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed Communicate effectively with clients, family members, and healthcare teams to update on patient status and coordinate care activities Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments with a focus on geriatrics or disability support Strong knowledge of dementia care, Alzheimer's care, memory care, hospice care, and neurodevelopmental disorders such as autism or intellectual disabilities Ability to observe patients carefully for changes in condition and respond promptly with appropriate interventions such as first aid or CPR when necessary Familiarity with care plans development and implementation tailored to individual needs; experience with patient monitoring and resident's rights is highly valued Skilled in assisting with ADLs, patient transfers using Hoyer lifts or other equipment, and manual handling techniques for heavy lifting safely Competent in meal preparation, personal transportation assistance, and supporting clients' participation in community activities Knowledge of HIPAA compliance standards for safeguarding patient information; experience working within home & community care settings preferred Excellent communication skills in both Spanish and English; ability to foster trusting relationships through compassionate caregiving Join our team if you're ready to bring your caregiving expertise into a role that truly makes a difference!
