Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health by taking vital signs and observing for any changes in condition Support medication administration following care plans and HIPAA regulations to ensure privacy and safety Prepare nutritious meals and assist with meal planning tailored to individual dietary needs Operate patient transfer equipment such as Hoyer lifts and manual handling techniques safely and effectively Maintain accurate documentation of patient care, observations, and incident reports in compliance with healthcare standards Provide companionship and emotional support, fostering a positive environment that respects residents' rights and dignity Skills Proven experience working with individuals with disabilities, developmental or neurodevelopmental disorders, or in senior care settings such as assisted living or nursing homes Strong knowledge of patient observation, monitoring vital signs, dementia care, Alzheimer's care, memory care, and hospice care practices Skilled in assisting with ADLs, personal transportation assistance, meal preparation, and manual handling techniques Familiarity with care plans, medication administration, HIPAA compliance, patient transfer methods, CPR/First Aid certification Excellent communication skills for engaging effectively with clients, families, and healthcare teams Ability to perform heavy lifting safely and manage patient transfers using proper equipment like Hoyer lifts Experience working within home health or community care environments is highly valued Join us to be part of a team that makes a real difference!
