Case Manager Program:
Family Mentoring Program (OCCTAC)
Pay Rate:
$20/hour (biweekly pay)
Schedule:
Part-time (minimum 20-25 hours per week)
Position Overview:
The Family Mentoring Program is seeking dedicated Case Managers to support families with children who have special needs. This role focuses on helping families become more self-sufficient by connecting them with appropriate community resources through a continuous referral and linkage process.
KEY RESPONSIBILITIES
Review and maintain confidential client records and files Build rapport with clients through phone and in-person communication Conduct home visits to assess family needs and monitor progress Identify and connect families with need-based resources and services Coordinate and consult on services tailored to each family's needs Prepare reports by collecting and summarizing client progress Ensure compliance with HIPAA and internal program regulations Perform other duties as assigned by supervisor
REQUIREMENTS
Proficiency in Microsoft Excel and Microsoft Office Strong verbal and written communication skills Bilingual in English and Spanish (preferred) Experience or knowledge working with children with special needs Valid driver's license and reliable transportation (must have a car) Must pass Live Scan background check Must provide proof of a negative TB test Ability to work a minimum of 20-25 hours per week
