Job Description

PRIMARY FUNCTION
The Family Case Manager provides family-centered case management, advocacy, resource navigation, and engagement services to youth and families served by the Boys & Girls Club of the Bellport Area (BGCBA). The position works across BGCBA departments to identify family needs, reduce barriers to services, strengthen connections among home, school, the Club, and community-based systems, and increase access to educational, behavioral, economic, and social supports. The Family Case Manager must have a strong working knowledge of K-12 school systems, special education services, Individualized Education Programs (IEPs), Section 504 Plans, behavioral supports, school-based interventions, Department of Social Services (DSS) programs, child care assistance, mainstream public benefits, and community resources. Reporting to the Chief of Staff, the Family Case Manager serves as a central point of coordination for family support across the organization and works collaboratively with SACC, Teen, academic, behavioral support, and other program staff. The position does not provide clinical treatment but assists families in identifying needs, navigating systems, accessing appropriate services, and coordinating supports. The Family Case Manager is also responsible for developing and implementing family activities, workshops, resource events, and community engagement initiatives that align with BGCBA's mission, strategic priorities, program goals and objectives, funding requirements, and the identified needs of the community.
FAMILY CASE MANAGEMENT & NEEDS ASSESSMENT
  • Conduct family-centered intake, assessment, and ongoing case management to identify family strengths, needs, barriers, and appropriate supports.
  • Develop individualized family goals and action plans for families requiring ongoing case management.
  • Maintain regular contact with families and document progress, referrals, follow-up, outcomes, and emerging needs.
  • Assist families in identifying and addressing needs related to education, child care, food security, housing, employment, health care, behavioral health, transportation, financial stability, and other factors affecting family well-being.
  • Provide families with information, advocacy, referrals, and hands-on assistance navigating service systems.
  • Follow up on referrals to determine whether families successfully accessed services and identify additional barriers when services were not obtained.
  • Utilize a strengths-based, family-centered, culturally responsive approach that respects family choice and promotes self-advocacy.
  • Participate in multidisciplinary case discussions and coordinate with appropriate BGCBA staff when a youth or family has needs affecting participation across multiple programs.
EDUCATIONAL ADVOCACY & SCHOOL-SYSTEM NAVIGATION
  • Maintain working knowledge of local K-12 school systems and educational resources available to students and families.
  • Assist parents and caregivers in understanding and navigating school policies, academic supports, attendance concerns, disciplinary processes, and school-based services.
  • Maintain working knowledge of special education processes, including referrals, evaluations, Committee on Special Education (CSE) processes, IEPs, Section 504 Plans, accommodations, related services, behavioral supports, and transition planning.
  • Help parents and caregivers understand their child's
IEP, 504
Plan, behavioral support plan, academic intervention plan, or other school-based services.
  • Assist families in preparing for parent conferences, CSE/504 meetings, behavioral planning meetings, and other school meetings.
  • With appropriate parent/guardian consent, collaborate with teachers, school social workers, counselors, psychologists, special education personnel, administrators, and other school-based professionals.
  • Help families connect school-based goals and supports with appropriate BGCBA programming.
  • Coordinate with BGCBA academic and program staff when educational needs affect a youth's participation or success within Club programming.
  • Refer families to educational advocacy or specialized services when needs exceed the scope of BGCBA services.
BEHAVIORAL & SOCIAL-EMOTIONAL SUPPORT COORDINATION
  • Work collaboratively with BGCBA behavioral support and program staff to identify youth and families who may benefit from additional behavioral, developmental, or social-emotional support.
  • Assist families in understanding behavioral support recommendations and connecting with appropriate school-based and community resources.
  • Support coordination of family components of behavioral support plans developed by qualified BGCBA staff, school personnel, or outside providers.
  • Connect families to behavioral health providers, counseling services, developmental services, crisis resources, parent support programs, and other appropriate services.
  • Participate in multidisciplinary case conferences involving youth with significant academic, behavioral, social-emotional, or family support needs.
  • Provide non-clinical family support, advocacy, problem solving, and resource navigation while recognizing when needs require referral to licensed clinical or other specialized professionals.
DSS, BENEFITS & RESOURCE NAVIGATION
  • Screen families, as appropriate, for potential eligibility for public benefits and other community resources.
  • Maintain current working knowledge of Suffolk County Department of Social Services (DSS) programs and other mainstream benefits.
  • Assist families in navigating and accessing child care assistance/subsidies, SNAP, TANF/Public Assistance, Medicaid, Child Health Plus, WIC, food and nutrition assistance, housing and emergency assistance, utility and energy assistance, transportation, employment/workforce services, disability and developmental services, foster/kinship resources, immigration-related community resources, and other local, county, state, and federal assistance programs.
  • Help families understand application processes, eligibility documentation, recertification requirements, and appropriate points of contact.
  • Assist families in connecting with agencies or benefits specialists when direct application assistance exceeds the scope of the position.
  • Maintain an up-to-date resource directory of government programs, nonprofit organizations, schools, health and behavioral health providers, and other community resources.
FAMILY & COMMUNITY ENGAGEMENT
  • Assess family and community needs through intake information, surveys, program data, participation trends, family feedback, and information from community partners.
  • Develop and implement an annual family engagement plan in collaboration with the Chief of Staff and program leadership.
  • Plan, coordinate, implement, and evaluate family activities and events that align with BGCBA's agency goals and objectives and respond to identified community needs.
  • Develop family programming that strengthens parent/caregiver engagement, supports youth outcomes, builds family connections to the Club, and increases access to resources.
  • Coordinate workshops and information sessions on IEP/504 navigation, school advocacy, behavioral supports, parenting, DSS and benefits navigation, financial literacy, food and nutrition, health and wellness, college/career readiness, and other topics identified by families.
  • Coordinate family resource fairs and opportunities for families to connect directly with schools, government agencies, health providers, nonprofit organizations, and community partners.
  • Collaborate with SACC, Teen, academic, arts, athletics, behavioral support, and other BGCBA staff to incorporate family engagement into program planning.
  • Support major BGCBA family and community events and ensure activities have clearly identified programmatic purposes and intended outcomes.
  • Develop community partnerships that expand resources and opportunities available to youth and families.
  • Create meaningful opportunities for parents and caregivers to provide feedback and participate in shaping programs and services.
  • Track participation and outcomes associated with family engagement activities and use data and family feedback for continuous program improvement.
SYSTEMS COORDINATION & COMMUNITY PARTNERSHIPS
  • Serve as a liaison among families, schools, government agencies, nonprofit organizations, health and behavioral health providers, and community partners.
  • Develop and maintain collaborative relationships with school districts, DSS, community-based organizations, health care providers, behavioral health organizations, food and housing resources, and other family-serving systems.
  • Help families involved with multiple systems coordinate communication, referrals, and services.
  • Represent BGCBA at appropriate school, community, coalition, and interagency meetings as assigned by the Chief of Staff.
  • Identify service gaps and emerging family needs and communicate trends and recommendations to the Chief of Staff.
  • Work with leadership to develop partnerships or programming responsive to identified community needs.
DOCUMENTATION, DATA & OUTCOMES
  • Maintain accurate, timely, and confidential case management records in accordance with BGCBA policies.
  • Document assessments, family contacts, referrals, services, follow-up, and outcomes.
  • Track family needs, referral patterns, service utilization, and barriers to accessing resources.
  • Track family engagement participation and outcomes.
  • Maintain appropriate releases of information before communicating with schools, providers, or other external organizations regarding individual youth or families.
  • Assist with data collection and reporting required by funders, government agencies, BGCBA leadership, and Boys & Girls Clubs of America.
  • Use case management and family engagement data to identify trends, service gaps, and emerging community needs.
  • Provide regular reports to the Chief of Staff regarding case management activity, family engagement, referrals, outcomes, and identified needs.
ORGANIZATIONAL COLLABORATION
  • Work collaboratively across BGCBA departments, including SACC, Teens, academics, behavioral support, arts, athletics, and administration.
  • Participate in multidisciplinary meetings to coordinate support for youth and families with complex needs.
  • Assist program staff in understanding family circumstances that may affect attendance, behavior, academic performance, or program participation while maintaining appropriate confidentiality.
  • Coordinate with program leadership to ensure family support services complement rather than duplicate existing program services.
  • Participate in supervision with the Chief of Staff, staff meetings, professional development, and agency-wide initiatives.
  • Perform other duties related to family support, community engagement, and organizational goals as assigned.
REQUIRED EXPERIENCE & QUALIFICATIONS
  • Bachelor's degree from a regionally accredited institution in Social Work, Human Services, Family Studies, Education, Psychology, Sociology, Public Administration, or a related field. Master's degree preferred.
  • Minimum of two years of relevant experience in family case management, social services, education, youth development, community-based services, or a related setting preferred.
  • Demonstrated knowledge of K-12 educational systems and school-based services.
  • Working knowledge of IEPs, Section 504 Plans, special education processes, behavioral supports, and educational advocacy.
  • Working knowledge of DSS, public benefits, child care assistance, SNAP, TANF, Medicaid/health insurance, food assistance, housing resources, and other mainstream benefits and community services.
  • Experience working with children, adolescents, and families with varying levels of need.
  • Experience coordinating family engagement activities, workshops, community events, or resource fairs preferred.
  • Strong advocacy, problem-solving, communication, and interpersonal skills.
  • Ability to build trusting relationships with families while maintaining appropriate professional boundaries.
  • Ability to work effectively across multiple departments and as part of a multidisciplinary team.
  • Strong organizational, case management, and documentation skills.
  • Ability to manage multiple families, projects, priorities, and deadlines.
  • Ability to maintain confidentiality and appropriately handle sensitive information.
  • Proficiency with Google Workspace and ability to learn agency data and case-management systems.
  • Ability to work a flexible schedule, including evenings and occasional weekends for family and community events.
  • Valid driver's license and insurance.
PREFERRED KNOWLEDGE & SKILLS
  • Knowledge of Suffolk County and Long Island family-serving systems and resources.
  • Familiarity with the South Country Central School District and other local school systems.
  • Experience in an out-of-school-time, school-age child care, youth development, or community-based organization.
  • Knowledge of trauma-informed, strengths-based, and culturally responsive family engagement practices.
  • Understanding of neurodiversity and the ways learning, developmental, behavioral, and social-emotional needs may affect youth and families.
  • Experience working within multidisciplinary teams.
  • Bilingual skills are a plus.
COMPENSATION & SCHEDULE
Pay Range:
$23.00-$25.00 per hour
Expected Hours:
Up to 35 hours per week Schedule generally includes Monday through Friday with day and afternoon/evening hours based on program and family needs. Evening and weekend availability is required periodically for family activities, community events, and agency programming.
