As directed, interview new residents and records, social history, and assessments. Completes the social services portion of the MDS, Care planning, baseline, etc. Refers to psychological services as needed. Schedules and attends and participates in interdisciplinary care plan meetings morning meetings, utilization review, etc. Writes summaries and progress notes as directed. Assists in conducting a pre-discharge evaluation including but not limited to setting up Visiting Nursing Services, resources, etc. in the community. Assists with discharge planning. Follows up after discharge. Conducts required Cut Letters Writes discharge summaries as assigned. Develops a professional relationship with each resident and family. Educates residents/ representatives on Advanced Directives. Adheres to all Social Services Regulations. Qualifications for a social worker Minimum Bachelor's Degree in Social Work from an accredited College. Must obtain a CSW and/or LSW. One to Three years of professional experience and/or training; or equivalent combination of education and experience. Join us as a social worker today! IND123
