Job Summary:
Responsible for coordinating the care of patients and their families or significant others within a level 1 Trauma Emergency room (ED) setting. Collaborates with local law enforcement, 414LIFE navigators, housing navigator, Milwaukee County Health Care Partnership program, 2-1-1, and other local community partners as appropriate. Monitors admission status, medical appropriateness, supports grieving families, and coordinates discharge Planning/end of life coordination. Has the ability to multitask multiple consults in a fast paced, high stress trauma ED environment 7 days a week.
EXPERIENCE DESCRIPTION
A minimum of 2 years of experience in an acute care setting is required. ED/Trauma experience and casework in medical social work is preferred.
EDUCATION DESCRIPTION
Master's Degree in Social Work.
SPECIAL SKILLS DESCRIPTION
Interpersonal skills that are necessary to obtain information from patients, to provide counseling to patients/families, and interact effectively with patients, families, and hospital personnel. Analytical skills associated with the patient's discharge and to be able to develop solutions to problems concerning patient's mental and physical wellbeing.
