Intensive Services Coordinator is responsible for providing support to individual/families served by Community Crossroads who have or are at risk of engaging in behavior that can cause harm to themselves or others, property destruction, and /or have complex mental health diagnosis and/or substance abuse. Intensive Services Coordinator functions through direct and ongoing contacts and assists individuals/families in identifying services which will provide the level of supervision, treatment, and environmental features that minimizes risk of harm to themselves and others. The goal being to assist the individual to enjoy quality of life experiences, have choice and control over services and supports and assist in planning and advocating for supports which promote dignity, respect and active participation. Provide flexible, individualized supports that focus on empowering families, individuals and communities and assures the safety of the individual, community, and caregivers.
SPECIFIC DUTIES
1. Advocate for needed services and supports on behalf of individuals and/or family for services. 2. Provide direct assistance to individuals/families as needed and as required by agency and state regulations. This includes home /program visits, telephone contact, team meetings, information & referral, monitoring quality of Community Crossroads contracted services, and accessing community services as well as local mental health services, when needed. 3. Facilitate service planning process and lead team meetings to assure meetings promote individual choice & control and address the needs of the individual/family. 4. Mediate concerns and issues among contracted service provider agencies, guardians, and other members of the individual team. 5. Develop a trusting relationship with individuals and families to foster open communication. 6. Facilitate and develop annual service agreements in accordance with state regulations for all individuals with community care waiver funded services. 7. Identify risk factors that may cause harm to individual or others and seek comprehensive risk assessment to identify appropriate level of supervision and treatment can be implemented through the Person-Centered Plan of Dynamic Risk Manageability. Provide enhanced and additional announced and unannounced visits to ITS level homes to ensure mitigations outlined in risk management plans and/or HRC approved behavior support plans are being implemented and followed. 8. Serve on the Community Crossroads Local Risk Management Committee to assist in identifying who has qualifying risk behavior, assure that the individual receives the appropriate support and services, and mitigate risk 9. Participate in Statewide Risk Management Committee meetings and Community of Practice meetings, when needed. 10. Understand individuals behavioral support needs and participate in the Behavior Review Committee (subcommittee of the Human Rights Committee) to present summary of individuals behavioral needs and respond to feedback and/or questions when needed, and/or serve as a BRC committee member. 11. Monitor implementation of funded services & progress through review of monthly documentation, program visits & monitoring, listening to input from family, guardian, individual, and/or provider of services and addressing any needed changes. 12. Assure individuals have knowledge of application process for benefit entitlements which are necessary for community care waiver funding to assure that all annual re-determinations are followed through on behalf of the representative payee. 13. Participate in and promote the importance of continuity of care by collaborating regularly with associated community mental health centers, private practices, hospitals, schools, and all other associated healthcare and mental health care professionals/organizations. 14. Assess the individuals need for services and complete necessary paperwork to assure individual is identified on the Wait List or Projected Service Needs List as appropriate and maintain contact with the individual or family regarding their status. 15. Assure that required agency and state documentation is completed and maintained in each individual/family file as record by agency and state regulations. 16. Assure that additional documentation in regard to intensive treatment services is completed as requested. 17. Assist families/guardians in the development of services. This includes development of the request for proposal, assuring the program proposal and budget meet the needs of the individual/family, transition planning, and facilitation of service start up. 18. Assure that the health, safety, and basic needs of the individual are being met and to report any allegation of abuse, neglect, or exploitation as required by agency policy and state regulations.
QUALIFICATIONS
Two years' experience working in human services with individuals or families or related field or Associate/Bachelors Degree in Psychology or Social Work field. An understanding and respect for the needs of individuals with chronic medical needs, an understanding of advocacy, self-advocacy, concepts of self-determination and person-centered services. Strong skills in the areas of communication, problem solving, conflict resolution, facilitation, advocacy, group process and mediation coordination and monitoring of services. Ability to empower individuals through modeling, encouragement, providing accurate information and offering non-judgmental support.
