Brethren Village Retirement Community, in Lititz, PA, is seeking a Social Worker for full-time employment. This is a great opportunity for a qualified individual with a Bachelor's degree in Social Work, at least 2 years of relevant experience in a mental health / social work field, and who will enjoy interacting with an older adult population. The Social Worker provides education, referrals, and supportive services to Residents and their family members, with the goal of promoting safe and independent living on the Brethren Village campus. The Social Worker will conduct assessments with Independent Living Residents; monitor and document changes in health status that may affect ability to safely live independently on our campus; coordinate appropriate services to support aging-in-place initiatives. The Social Worker also offers guidance and support to Residents and their family members during hospitalizations and transitions to higher levels of care, and provides resources and assistance to Independent Living Residents in a wide variety of situations. The successful applicant will be skilled in crisis response, and able to effectively de-escalate high-tension situations and stabilize individuals experiencing acute distress. The Social Worker completes all work with the highest degree of integrity and in compliance with HIPAA regulations. Work Hours Full-time, benefits-eligible: 40 hours per week, typically 8:00 a.m. - 4:30 p.m. or similar hours, Monday through Friday, on-siteOccasional evening or weekend hours may be required to address urgent situations
