JOB SUMMARY
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery. The Prototypes Pre-Filing Diversion Program, a program of HealthRIGHT 360, is part of a Los Angeles County-wide initiative, seeks to reimagine the justice system by diverting individuals arrested for misdemeanors and non-serious, non-violent felonies from criminal charges to community-based support services. ATI addresses key factors leading to incarceration, including homelessness, mental health, addiction, and lack of employment opportunities. Through collaboration with law enforcement and district attorneys, PFDP is working to build a sustainable network of providers that enhances the community's ability to support those in need. The Systems Navigator plays a critical role in supporting individuals detained by law enforcement, conducting screenings and assessments to determine eligibility for diversion services. This in-person role connects clients to housing, healthcare, substance use treatment, and other social services through our network of partner agencies, providing trauma-informed, culturally competent guidance and advocacy. By offering motivation, support, and practical resources, the Systems Navigator empowers clients to build stability, access community-based services, and reduce the risk of incarceration. This position is co-located at the Los Angeles Police Department (LAPD) Metro Detention Center. This role requires a reliable transportation.
KEY RESPONSIBILITIES
Client Engagement & Support Conduct engagement and orient new participants to the program and its expectations. Complete screening, intake, assessment, and enrollment; identify strengths and needs. Provide empathetic, trauma-informed, and culturally competent support. Create transition plans for clients released from custody and coordinate transportation. Provide housing navigation services and link participants to mental health, addiction treatment, benefits, employment, education, childcare, medical, and other services. Coordinate with Peer Navigator/Driver for client transportation needs. Provide crisis intervention and connect clients to urgent services as needed. Offer life skills coaching to help clients build practical skills for daily living and independence. Documentation & Compliance Maintain accurate and timely documentation at a high standard. Collect pre- and post-surveys and outcome measures. Maintain confidentiality in compliance with agency, HIPAA, and 42-CFR standards. Participate in ongoing professional development, including trauma-informed care, cultural competency, and motivational interviewing. Collaboration, Outreach & Safety Interface with multidisciplinary team in treatment planning and service delivery. Participate in team meetings, case conferences, and supervision. Conduct outreach and maintain referral pathways with community partners and service providers. Adhere to HealthRIGHT 360's Health & Safety policies, including emergency preparedness, hazard reporting, and proper emergency response protocols. And perform other duties as assigned.
QUALIFICATIONS
Education, Certification, or Licensure Option I o High school diploma or equivalent. o A minimum of three (3) years' experience providing case management experience working with justice-involved individuals and families. Option II o Bachelor's degree in Social Work, Psychology, or related field. o A minimum of two (2) years' experience providing case management experience working with justice-involved individuals and families. Current First Aid and CPR certification or ability to obtain within 30 days of hire. Valid California Driver's License and access to registered and insured transportation. Experience Experience in working with families from low SES communities, diverse cultural ethnic, and linguistic backgrounds. Experience working with individuals with mental health, substance use, domestic violence, and/or cognitive challenges.
