Key Responsibilities:

Perform comprehensive home health risk assessments, averaging 45 minutes each, for Medicare Advantage members Evaluate patients prior/current medical histories, chronic conditions, and medications Educate patients about their health conditions and prescribed treatments Identify care gaps, document findings, and provide recommendations No requirement for treatment plan creation or medication prescribing

Desired Qualifications:

Active Nurse Practitioner license in North Carolina Current CPR (BLS) certification Board Certified through AANP or

ANCC MSN

from an accredited institution Minimum six months of recent Nurse Practitioner experience preferred
