We are seeking a dedicated Family Practice Nurse Practitioner for an 80-day assignment in Maine, commencing Feb 1, 2027. This role involves working 4 10-hour shifts, providing full scope primary care, chronic care management, and supporting walk-in clinic needs, with a significant focus on a geriatric patient population and comfortable management of chronic medications. Responsibilities and Duties Provide comprehensive family medicine care in a walk-in setting. Manage a patient panel with a focus on geriatric and older populations. Deliver full scope primary care services. Conduct chronic care management and medication management. Offer walk-in clinic support, including minor urgent care cases. Comfortably manage patients on chronic medications. Utilize onsite lab and X-ray services. Handle post-discharge follow-ups from the Emergency Department. Address higher acuity cases, typical for an older patient demographic. Additional Information Board Certification in Family Medicine is required. The facility utilizes Meditech EHR.
