The individuals served in this home use wheelchairs and have limited mobility due to physical disabilities. This position requires extensive personal care assistance, including transfers, bathing, toileting, incontinence care, and maintaining a clean and safe living environment. The ideal candidate is patient, respectful, reliable, and committed to improving the quality of life of those in their care. Responsibilities Complete accurate and detailed daily documentation, including progress notes, medication records, bowel movement (BM) tracking, health observations, incidents, and all services provided. Document all care activities in compliance with Guardian Residential Services and DBHDS requirements. Ensure documentation is completed timely, thoroughly, and professionally, as it is a critical part of the position. Requirements Experience with healthcare or residential documentation strongly preferred. Must be able to write clear, accurate, and detailed progress notes and follow documentation standards. Strong attention to detail and understanding of the importance of documentation for compliance and quality care. Comfortable using electronic or paper documentation systems. Prior experience as a DSP, CNA, HHA, Caregiver, or in a related direct care role preferred. Important Notice This position involves extensive hands-on personal care, including assisting individuals who use wheelchairs and have paralysis.
