As a Community Outreach & Care Manager, you'll serve as the bridge between clients, families, referral partners, and the EverTrue Anywhere Care team. This hybrid position combines clinical social work with healthcare outreach, helping older adults and their families navigate care options while building strong relationships throughout the community. In this role, you'll support clients and referral partners across several Anywhere Care programs, including: GUIDE

• Supporting individuals living with dementia and their caregivers through the Guiding an Improved Dementia Experience (GUIDE) Model. At Home+
• Helping older adults remain safe and independent at home through personalized care management and support services. Home Help (Private Duty)
• Connecting clients with non-medical in-home care services that promote independence, safety, and peace of mind. Additional Anywhere Care programs
• Serving as a trusted resource by connecting clients and families with the full continuum of EverTrue services based on their individual needs.

Working closely with hospitals, physicians, senior living communities, case managers, and other referral partners, you'll ensure older adults receive the right care at the right time while helping grow EverTrue's community-based services throughout the St. Louis metro area. Required Qualifications Applicants must meet the following qualifications: Bachelor of Social Work (BSW) or Master of Social Work (MSW) required Previous healthcare outreach, community outreach, or healthcare liaison experience required Experience building relationships with healthcare providers, hospitals, physicians, senior living communities, or other referral sources Excellent communication, relationship-building, and presentation skills Comfortable working independently while managing multiple community relationships Ability to travel throughout the St. Louis metro area What You'll DoSupport Clients & Families Conduct psychosocial assessments and develop individualized care plans. Guide clients and families through care decisions, transitions, and long-term planning. Identify barriers to care and connect clients with appropriate resources. Educate families on EverTrue Home Help, At Home+, GUIDE, and other available services. Manage new client enrollments and execute service agreements. Build Community Partnerships Develop and maintain relationships with hospitals, discharge planners, physicians, clinics, skilled nursing facilities, senior living communities, elder law attorneys, fiduciaries, and community organizations. Serve as a trusted clinical resource for referral partners by educating them on EverTrue Anywhere Care services. Represent EverTrue at networking events, educational programs, and community meetings. Facilitate seamless handoffs between referral partners and intake teams to ensure timely service delivery. Grow the Anywhere Care Program Build trust with prospective clients and families while supporting appropriate growth of private duty services. Track referral activity, outcomes, and pipeline progress in the CRM. Participate in weekly sales and marketing meetings and collaborate on quarterly growth strategies. Maintain knowledge of Medicare, Medicaid, insurance coverage, and EverTrue's continuum of care. Why Join EverTrue?
