Job Description Home Health Links is hiring Physical Therapists (PTs) for Start of Care (SOC) visits only across Los Angeles and Ventura County . We're a therapist-owned home health agency with over a decade of leadership in Southern California—and we use AI-driven OASIS technology that cuts documentation time from 90 minutes to about 15 minutes , so you can focus on patient care, not paperwork. Responsibilities:
Assess the therapeutic and rehabilitative status of the patient Develop an appropriate treatment plan for each patient; update and modify treatment plan on an ongoing basis. Ensure high-quality patient care Use clinical reasoning in treating and planning, implementing, and monitoring patient progress Maintain records with accuracy and discretion We set standard base pay ranges for all roles based on function and level. Final offer is determined by multiple factors, including, skillset, work experience and languages, and may vary from the amounts listed below. The SOC visit pay range for this position is: $130•145/visit. Qualifications Complete Start of Care (SOC) assessments for new patients Degree from an accredited Doctorate of Physical Therapy (DPT) program. Current and unrestricted CA PT license; or ability to apply Excellent Communication Skills Home Health experience preferred; not required Additional information All your information will be kept confidential according to EEO guidelines.
