Legacy Home Care and Home Health Care, Inc. are hiring PTs to provide home health care throughout Bethel, Cedar, East Bethel, Oak Grove, and St. Francis . This is an opportunity to step outside the traditional treatment environment and use your clinical judgment to solve the functional problems patients are facing in everyday life. You're Not Treating a Diagnosis. You're Figuring Out What Is Keeping Someone From Functioning. A patient may have been discharged after a hospitalization with instructions to keep moving. Then you arrive at their home. There are three steps at the entrance. Their walker doesn't fit easily through the hallway. They're struggling to stand from their favorite chair. Their spouse is nervous about assisting with transfers. Suddenly, the clinical picture looks different. Home health lets you see the entire problem. You'll evaluate what's happening, establish the plan of care, determine appropriate interventions, monitor progress, and adjust the plan as the patient's needs change. You'll:
Complete in-home Physical Therapy evaluations and treatment visits Complete admissions and start-of-care assessments Develop individualized plans of care Address strength, balance, gait, mobility, transfers, and endurance Evaluate fall risks and home safety concerns Identify barriers to functional independence Educate patients, families, and caregivers Supervise and collaborate with Physical Therapist Assistants Coordinate care with nurses, OTs, SLPs, physicians, and other clinicians Monitor patient progress and modify plans of care Complete timely clinical documentation and OASIS assessments when applicable Your Background Doesn't Have to Be Home Health We're interested in PTs from Acute Care, Inpatient Rehabilitation, Outpatient Orthopedics, Skilled Nursing, Transitional Care, Senior Living, Long-Term Care, Hospital-Based Rehabilitation, and Home Health . Previous home health and OASIS experience are preferred, but they aren't required . What matters is your ability to assess a patient, think critically, communicate effectively, develop an appropriate plan, and work confidently in an independent environment. You'll need a current Minnesota Physical Therapist license , valid driver's license and reliable transportation, and availability for at least 2 weekdays / 12 visits per week . A Defined Northern Metro Coverage Area This position serves patients throughout Bethel, Cedar, East Bethel, Oak Grove, and St. Francis . You'll travel between patients within this coverage area, working one-on-one with people who need Physical Therapy to safely function at home. You Became a Physical Therapist to Figure Out How to Get People Moving Again. Not every problem can be solved with another set of exercises. Sometimes you need to see the patient stand from their actual chair. Watch them navigate their actual stairs. Understand what their caregiver can realistically manage. See what happens when they try to move through their normal day. That's where your clinical judgment becomes incredibly valuable.
