This is mobile outpatient physical therapy—not traditional home health . You'll provide individualized, evidence-based care without juggling multiple patients, working under productivity quotas, or completing OASIS documentation. This position is ideal for a PT who values autonomy, meaningful patient relationships, and the flexibility to create a schedule that fits their life. It's a great opportunity for someone looking to supplement an existing work schedule, with the potential to gradually build toward a full-time caseload if desired. Schedule Must have availability during at least two days per week; full-day availability is not required Opportunity to add patients and additional days over time Freedom to arrange visits around your existing schedule Patients grouped within a compact geographic area to minimize driving Responsibilities Evaluate and treat patients in their homes Develop individualized plans of care based on each patient's goals Provide orthopedic, postoperative, balance, fall-prevention, strength, mobility, and neurological rehabilitation Complete timely and accurate clinical documentation Communicate effectively with patients, families, physicians, and other healthcare providers Manage visits independently while delivering attentive, patient-centered care

Qualifications Required:

Current Nevada physical therapist license in good standing Graduate of an accredited physical therapy program Reliable transportation and willingness to travel locally within Henderson Strong communication, organization, time-management, and documentation skills Ability to work independently and provide high-quality, one-on-one care

Preferred:

Experience with older adults, orthopedic conditions, balance and fall prevention, neurological rehabilitation, or postoperative care Experience working independently in a mobile, outpatient, or home-based setting Familiarity with Medicare documentation and requirements Medicare experience is helpful but not required.
