Mercy is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team. In this role, you will assess, plan, and implement individualized therapy programs to restore function, improve mobility, and relieve pain for patients across diverse conditions. Responsibilities Evaluate patients in an outpatient setting using standardized tests, functional assessments, and clinical observation to determine impairments, functional limitations, and rehabilitation goals. Develop and implement individualized, evidence-based plans of care for a diverse outpatient caseload, including orthopedic, post-surgical, neurological, and chronic pain conditions. Provide hands-on manual therapy, therapeutic exercise, neuromuscular re-education, and appropriate modalities to restore function, reduce pain, and improve mobility. Educate patients and families on diagnosis, home exercise programs, injury prevention, and long-term self-management strategies to support sustained outcomes. Collaborate closely with physicians, nurses, occupational therapists, and other members of the interdisciplinary team to coordinate comprehensive patient care. Accurately document evaluations, daily treatment notes, progress reports, and discharge summaries in the Electronic Medical Record (EMR) in a timely and compliant manner. Monitor patient progress, adjust treatment plans based on clinical findings and patient response, and communicate changes to the care team and referring providers. Adhere to all Mercy policies, state and federal regulations, and professional standards related to safety, infection control, ethical practice, and patient confidentiality. Participate in performance improvement initiatives, case discussions, and continuing education to maintain and advance clinical competency in outpatient physical therapy. Provide mentorship, guidance, and clinical support to students or less experienced staff as assigned, contributing to a collaborative learning environment.
