Job Description
Job Summary We are seeking a compassionate and dedicated Registered Nurse - Home Health to provide high-quality patient care in the comfort of their homes. The ideal candidate will possess strong clinical skills and the ability to work independently while collaborating with a multidisciplinary team. This role involves delivering nursing care, educating patients and their families, and ensuring adherence to treatment plans. Duties Conduct comprehensive patient assessments and develop individualized care plans. Administer medications and treatments as prescribed, including ostomy care and tube feeding. Monitor patient progress and report any changes in condition to the healthcare team. Maintain accurate medical records using eClinicalWorks software. Educate patients and families on disease management, medication administration, and home community care practices. Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care. Provide support for outpatient procedures such as cardiac catheterization when necessary. Participate in quality improvement initiatives to enhance patient outcomes. Experience Current Registered Nurse (RN) license in the state of practice. Previous experience in a clinic or teaching hospital setting is preferred. Familiarity with home health care practices and standards is highly desirable. Proficient in using electronic medical records systems, particularly eClinicalWorks. Experience with cardiac catheterization, ostomy care, and tube feeding is a plus. Strong communication skills and the ability to teach patients effectively. Ability to work independently while managing multiple priorities in a home health environment.
