Superior Hospice is looking for an RN Case Manager to care for patients throughout the Northwestern Greater Twin Cities Metro Area . Instead of spending your entire day in one building, you'll bring skilled nursing directly to patients wherever they call home. A Different Kind of Nursing Day Hospice case management isn't about sitting behind a desk managing a list of patients. You're in the community. You'll visit patients in their homes, Assisted Living communities, Memory Care settings, and Skilled Nursing facilities. You'll assess changing needs, manage symptoms, educate families, collaborate with providers, and coordinate the bigger picture of each patient's care. Your patients change. Your environment changes.

What You'll Do Manage an assigned hospice caseload throughout the Northwestern Greater Twin Cities Metro Area Complete comprehensive nursing assessments Develop and update individualized plans of care Manage pain, dyspnea, agitation, wounds, medications, and other symptoms Recognize changes in condition and coordinate interventions Educate patients and caregivers about symptoms, medications, disease progression, and comfort measures Communicate with physicians, pharmacies, facilities, and community providers Collaborate with hospice aides, social workers, chaplains, and other members of the interdisciplinary team Provide care across private homes and senior living settings Maintain accurate and timely clinical documentation Advocate for each patient's comfort, dignity, and individual goals of care You May Be More Prepared for Hospice Than You Think We're interested in experienced nurses from Home Health, Palliative Care, Oncology, ICU/Critical Care, Med/Surg, Skilled Nursing, Long-Term Care, Assisted Living, Memory Care, and Community-Based Nursing . Hospice experience is valuable, but so are strong assessment skills, clinical confidence, organization, patient education, and the ability to work independently. What We're Looking For Active Minnesota Registered Nurse (RN) license Strong clinical assessment and critical-thinking skills Ability to independently manage and prioritize a patient caseload Excellent patient and family communication Strong organization and care coordination Confidence working independently in the community Valid driver's license and reliable transportation Willingness to travel throughout the Northwestern Greater Twin Cities Metro Area Hospice experience preferred, but strong transferable experience is encouraged Maybe You Don't Need a Different Career.
