The Licensed Social Worker (LSW/LCSW) is responsible for providing high-quality clinical and case management services to individuals receiving treatment for substance use disorders and co-occurring mental health conditions. The Social Worker will work as part of a multidisciplinary treatment team to assess clients' needs, develop and implement individualized treatment plans, provide individual and group counseling, coordinate services, facilitate referrals, and support clients throughout the recovery process. Essential Duties and Responsibilities Clinical Services

• Conduct comprehensive psychosocial assessments and gather information regarding clients' substance use, mental health, family, social, educational, employment, and legal histories.
• Participate in the development, implementation, and ongoing review of individualized treatment plans.
• Provide individual, group, and family counseling as appropriate to the level of care and professional credentials.
• Utilize evidence-based, trauma-informed, and person-centered approaches when working with clients.
• Assist clients in identifying strengths, barriers to recovery, triggers, coping skills, and relapse-prevention strategies.
• Provide crisis intervention and appropriate clinical support when necessary.
• Monitor client progress and modify treatment interventions based on changing needs.
• Identify signs of mental health deterioration, relapse risk, or other clinical concerns and communicate appropriately with the treatment team.
• Coordinate with counselors, case managers, medical providers, psychiatric providers, and other members of the multidisciplinary team. Substance Use Disorder Treatment
• Provide education regarding substance use disorders, addiction, recovery, relapse prevention, and harm reduction.
• Assist clients in understanding the relationship between substance use, mental health, relationships, and other life circumstances.
• Help clients develop practical recovery skills and establish healthy support systems.
• Encourage participation in community-based recovery supports and appropriate peer-support programs.
• Assist with discharge planning and development of continuing-care and aftercare plans. Case Management & Care Coordination
• Assist clients with identifying and accessing appropriate community resources.
• Coordinate referrals for mental health treatment, medical care, housing, employment, education, transportation, benefits, and other social services.
• Collaborate with outside providers and agencies, when authorized by the client, to promote continuity of care.
• Help clients overcome barriers that may interfere with treatment participation and successful recovery.
• Participate in discharge planning and facilitate appropriate transitions to continuing-care services. Documentation & Compliance
• Maintain accurate, timely, and clinically appropriate documentation in accordance with facility policies and applicable regulations.
• Complete assessments, treatment plans, progress notes, discharge summaries, and other required clinical documentation.
• Maintain client confidentiality in accordance with HIPAA and applicable federal and state regulations.
• Follow all facility policies, procedures, ethical standards, and clinical protocols.
• Participate in quality assurance and performance improvement activities.
• Maintain all required licenses, certifications, continuing education, and professional training. Team Participation
• Participate in treatment team meetings, case consultations, supervision, and staff meetings.
• Communicate effectively with clinical, medical, administrative, and support staff.
• Advocate for clients while maintaining appropriate professional boundaries.
• Contribute to a safe, respectful, supportive, and recovery-oriented treatment environment.
• Assist with program development and community outreach activities when appropriate. Qualifications
• Required
• Master's degree in Social Work (MSW) from an accredited program.
• Current New Jersey Social Work license appropriate to the position (LSW, LCSW, or applicable credential).
• Experience working with individuals with substance use disorders and/or co-occurring mental health disorders preferred.
• Knowledge of substance use disorder treatment principles and recovery-oriented practices.
• Strong written and verbal communication skills.
• Ability to maintain professional boundaries and protect confidential client information.
• Ability to work effectively as part of a multidisciplinary treatment team. Preferred
• Experience in a substance use disorder treatment, behavioral health, outpatient, residential, or inpatient setting.
• LCSW clinical licensure.
• Experience with co-occurring disorders, trauma-informed care, and crisis intervention.
• Knowledge of evidence-based therapeutic approaches such as Motivational Interviewing, CBT, DBT, and relapse-prevention interventions.
• Experience facilitating psychoeducational or therapeutic groups.
• Experience with electronic health records and behavioral health documentation.
