  • Health Information Services plays a key role in organizing, tracking, and reporting health information for both residents and staff of the nursing home.

This position supports the Medical Director and Director of Nursing in ensuring the accurate and confidential handling of medical records and related data, while maintaining compliance with state regulations and healthcare standards. Examples of Duties 1.

Medical Records Management:
  • Maintain resident medical records in compliance with State of New Hampshire regulations.
  • Transcribe dictation from the Medical Director and attending physicians, including discharge summaries for deceased or discharged residents.

Physician Support:
  • Coordinate and organize workflow to support timely completion of documentation required for regulatory compliance and billing processes.
  • Ensure medical records are accurately prepared and available for review and sign-off.

3.

Chart Review and Filing:
  • Review charts for completeness upon resident discharge or death.
  • File charts once verified and signed by the physician.

4.

Information Processing and Documentation:
  • Develop and maintain systems for receiving, scanning, and storing protected health information (PHI).
  • Uphold strict confidentiality standards in the handling of PHI.

5.

Resident Admission Coordination:
  • Compile information for all new admissions.
  • Notify departments of new resident arrivals, prepare necessary labels for documentation, and update the physician 60-day note schedule as needed.

6.

Employee Health Tracking:
  • Collect and manage confidential staff information related to infection control, screening, and follow-up testing.
  • Assist the Infection Preventionist with scheduling staff for testing and vaccinations, and report data to appropriate authorities.

MDS Support:
  • Assist with the input of Minimum Data Set (MDS) and Resident Assessment Protocol (RAP) summaries as needed. Additional Information Help for Additional Information. Additional Information Help for Additional Information. Typical Qualifications
  • High school diploma or equivalent with demonstrated experience in office procedures and systems, including typing skills.
  • The incumbent should have up to three (3) years office administration experience. An Associates Degree in Office Management, Business Science or Medical Assistant Records could substitute for two years of experience.
  • Knowledge of Business English, and/or medical terminology.
  • Experience with modern office equipment, practices and procedures, and basic elements of personnel management record keeping.
  • Knowledge of arithmetical computations and ability to make tabulations with speed and accuracy.
  • Ability to take and transcribe dictation, keep complex clerical records and prepare accurate reports from such records.
  • Ability to prepare effective correspondence on routine matters and perform routine office management details without referral to a supervisor.
  • Ability to establish and maintain effective working relationships with other employees and the general public.
