$26.3K – $33.8K a year 11 hours ago Qualifications High school diploma or GED Entry level Office experience Full Job Description Jacksonville, FL 32257 Care Navigation Center (CNC) Documentation Specialist organizes, communicates and documents information related to the hospice certifications according to state and federal regulations. Obtain attending/referring MD and CHNE MD signatures on hospice certifications within 2 days of admission. This may require travel to the physicians’ office to obtain the signature. Document signatures in Suncoast Update patients chart with signed hospice certification. Coordinate with other teams regarding the certification procedure. Ensuring consents are accurately signed and documented.

Education/Experience:

Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to: High School Diploma or equivalent A minimum of 1-year experience in an office environment Experience in a health care environment preferred We are an equal opportunity employer.
