Position Summary The Housing Stabilization Specialist (HSS) plays a vital role in supporting families referred to Rapid Rehousing (RRH) services to quickly exit homelessness and achieve long-term housing stability. This position provides short-term financial assistance and housing-focused case management to help prevent return to homelessness. Working in the Prevention and Diversion Services department, the Specialist assists a maximum of twenty-five (25) families, who are seeking placement through the City of Philadelphia’s Office of Homeless Services. Essential Duties and Responsibilities Client Engagement & Intake Schedule and complete initial interviews within 10 business days of referral. Conduct HUD Entrance Assessments to determine eligibility for RRH services. Complete intake assessments to identify barriers to housing, such as income, employment, credit history, mental/physical health, and legal history. Complete a financial screening to determine eligibility and level of financial assistance needed. Service Planning & Housing Support Manage a caseload of 15-20 active households. Complete housing assessments and co-develop individualized Rapid Rehousing plans with participants. Collaborate with the Housing Counselor to address tenant barriers and identify appropriate housing. Assist with housing search and maintain updated listings of available units in collaboration with the Housing Counselor. Support participants with housing applications, landlord communication, and lease-up processes. Conduct home visits and assist with monthly budgeting and financial planning. Coordinate ongoing communication with landlords/property management to support tenancy. Support conflict resolution and landlord mediation as needed. Service Navigation & Collaboration Provide resource navigation and referrals to services including behavioral health, employment, benefits access, DV support, and credit counseling. Participate in community outreach and assist with public education about RRH services. Collaborate with internal HopePHL staff and external partners to ensure continuity of care and goal alignment. Landlord and Partner Coordination Maintain proactive, ongoing communication with landlords and property managers to resolve housing issues, negotiate payment arrangements, and coordinate support. Collaborate with internal staff and external partners to connect clients with wraparound services (e.g., legal assistance, employment support, childcare, and behavioral health services). Conduct regular follow-ups with clients and landlords to ensure the timely submission of all necessary paperwork. Retrieve and organize required documentation from clients, landlords, and third parties to support service delivery and eligibility verification. Documentation & Compliance Document all client interactions, progress notes, and service activities in Cares.net and other required databases in a timely and accurate manner. Maintain audit-ready client files in compliance with program and funder requirements. Track progress toward individual and programmatic goals and ensure contractual deliverables are met. Ensure compliance with agency, legal, and ethical standards for confidentiality, data security, and client rights. Professional Expectations Attend supervision bi-weekly and arrive prepared with updated case documentation. Share weekly schedule with supervisor and maintain regular communication. Attend all required team and agency meetings and adhere to agency attendance policies. Perform other duties as assigned.

Work Environment:

This position is a hybrid (three days in the office, two remote), with flexible schedule options. Occasionally travel to the offices of foundation officials or off-site locations will be required. The incumbent in this position will routinely use computers, photocopiers, filing cabinets and other standard office equipment. The position also requires you to assist with large gift in-kind drives, and occasional event set up and breakdown.

Physical Demands:

While performing the duties of this job, the employee is frequently required to sit, talk, and hear. The employee is occasionally required to walk, use hands and fingers to feel, handle, or operate objects or controls; and reach with hands and arms. The employee must occasionally lift and/or move up to 30 pounds.
