Description:
The Veteran System Intake Specialist works with the Pinellas County Continuum of Care (CoC), partner agencies, and homeless outreach teams to conduct eligibility and priority screening of Veteran households to determine program eligibility. The primary goal is to identify unsheltered homeless Veterans in conjunction with street outreach teams and through community referrals from mental health providers, hospitals, local law enforcement, municipalities, veteran administration, and other social service agencies to connect with housing and shelter opportunities. The Veteran System Intake Specialist will also screen veterans at risk of homelessness and triage referrals accordingly.
Specific Requirements:
Works collaboratively with the Veteran System Navigator, Senior Manager of Coordinated Entry, and Veteran Service Specialist to promote Coordinated Entry in the community, emphasizing the focus on those who are most vulnerable. Administer the Family Shelter Assessment (FSA) and enroll literally homeless veteran families into the Shelter Prioritization List Program Entry. Assists with administering the Coordinated Entry Assessment for all homeless veterans, as needed. Facilitate Veteran “By-Name-List” (Case Conferencing) Meetings with providers and stakeholders in the community. Assists and participates in all Veteran-related meetings, including but not limited to Grant-Per-Diem (GPD), and SSVF/VA Collaborative Meetings. Assign prioritized Veterans and Veteran families referred through CE for the purposes of document readiness. Assist other Navigators and case managers with troubleshooting of housing readiness documentation and collection for veterans, individuals, and families experiencing homelessness. Assists in identifying appropriate housing opportunities when clients have not yet been assigned a case manager and helps to complete housing applications when necessary. Provides support and direction for Veterans and families as they transition from homelessness to housing. Provide light-touch case management to at-risk participants and/or participants experiencing Category 1 Literal Homelessness. Works with agency-based navigators and the outreach teams from other agencies in Pinellas County to locate and keep track of the location of the most vulnerable veterans, individuals, and families to allow for quicker entry into housing. Maintains statistics and completes administrative reports. Participates in the process of program planning as appropriate and requested by the Senior Manager of Coordinated Entry. Maintain complete client records, daily activity logs, and other reports as directed. Assists with the maintenance, including sorting, upkeep, and review of the housing priority list. Specifically including veteran by name list and updates from veteran providers. Participates and provides meaningful contributions when the team or agency explores ways to improve the program and/or its workflow. Organizes work, establishes priorities, and exemplifies adaptability to change and creative thinking to improve quality, productivity, and customer service/satisfaction. Participate in Emergency Management activities as needed. Excellent communication skills, particularly listening, mediation, and writing skills. Education:
Bachelor’s degree (BA/BS) preferred from a four-year college or university in human services, social work, public administration, social sciences and at least two years of experience in serving homeless or at-risk families and or individuals in crisis is required. Must have working knowledge of Veteran Services, Coordinated Entry, Diversion, Housing First, Prevention, Rapid Re-Housing, Permanent Supportive Housing, and Trauma Informed Care philosophies and practices.
Language Ability:
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
Math Ability:
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability:
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
Computer Skills:
To perform this job successfully, an individual should have knowledge of Microsoft suite, excel, website updates, and Microsoft 365 cloud.
Knowledge, Skills and Abilities:
Knowledge of unsheltered communities and issues with an emphasis on unsheltered public policy and program evaluation. Knowledge of Veteran services, emphasis on homeless programs. Basic understanding of severe mental health, physical health, and substance abuse issues and symptoms. Ability to work effectively with people of varying racial, ethnic, cultural, educational, age, gender identity, sexual orientation, and socioeconomic backgrounds. Professionalism.
