The Outreach Specialist plays a key role in assisting clients with Marketplace insurance and Georgia Gateway programs (MEDICAID, SNAP, TANF, WIC, CAPS). The Outreach Specialist will work to provide outreach opportunities for community residents who lack access to healthcare and health insurance, provide enrollment and application follow-up assistance and build/strengthen enrollment collaborations with local partner organizations. The Outreach Specialist provides social services support to individuals and families in helping them connect with resources to help improve their daily lives and to utilize the services available at the health center as well as available resources within the community. Responsibilities and Duties Partnership Development. Works closely with the health center leadership in developing strategic plans for effective outreach. Works with and serves diverse families with an understanding of their needs. Builds and maintains strategic partnerships related to food, shelter, and medical access. Nurture relationships with community partners, churches, school staff, parents, volunteers, and service providers. Participate in community partners outreach events. Provide Social Services Assistance Assist clients with applying for SNAP, Medicaid, Child Care and TANF Benefits through Georgia Gateway. Assess client needs and assist with finding the resources needed. Receive telephone encounters and referrals from FCPC providers and assist their patients and families with finding the resources they need. Make referrals for patients and clients within the community seeking assistance. Make follow-up calls to patients and clients on all referrals made and applications completed for outcomes and to make sure no further assistance is needed. Complete Gateway training. Enter all referrals and telephone encounters into the EHR System Maintain accurate documentation on all patients and clients regarding services and referrals provided by the outreach department. Complete a monthly report on all services provided

Marketplace Insurance, Outreach and Enrollment:

Provide enrollment assistance (including but not limited to completing coverage applications, gathering required documentation and troubleshooting the enrollment process) for uninsured consumers to access subsidized, low-cost and free health insurance programs through the health insurance marketplace, Medicaid and the Children’s Health Insurance Program (CHIP). Provide structured patient education on health coverage, engage in follow-up conversations and offer renewal assistance for enrolled individuals. Distribute outreach materials to patients, community members, partner organizations and businesses to build coverage option awareness. Develop, discover and attend community events to promote coverage options and the mission and services of the organization. Collaborate with various local organizations to build awareness of coverage options, spur enrollment and build referral links. Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities. Accurately provide required reporting to track goal achievement and client satisfaction. Assist in the development and implementation of organizational outreach and enrollment initiatives. Distribute educational materials on health education, health awareness, and health insurance to all sites. Drop of donations and requested materials to community partners upon requests. Must be able to do a presentation in front of groups about FCPC, the services provided and other related information. Meets confidentiality and privacy standards. Creates a physical space where consumers can freely discuss their financial and personal situation without concern of being overheard. Maintains the highest ethical standards regarding non-disclosure of protected personal information. Sets up monitoring systems to ensure that only required data is stored and that it cannot be accessed without proper authorization. Other duties as assigned. Qualifications Requires at a minimum an associate degree. Four Years of customer service experience. Ability to effectively develop and nurture relationships with a diverse group of stakeholders. Ability to work independently and coordinate multiple tasks. Ability to enthusiastically communicate with potential enrollees. Strong computer skills with proficiencies in Outlook, Word, PowerPoint, Excel, internet-based applications and the Microsoft operating system. Typical Working Conditions The individual works inside the office to provide privacy to the consumer as well as outside the office at community events to perform outreach activities. Some after hours and weekend hours required subject to participating in community events. Some travel required based on training requirements. Some physical demands are required in transporting boxes of marketing materials and the setting up of booths.
