Population Health Specialist

Read Boulevard, New Orleans, LA 70127 p Population Health Specialists serve as part of a teambrresponsible for assisting patients with timely and appropriate access care withbrPrimary Care and/or Specialty providers within our network of clinics. Facilitation of inbound and outboundbrtelephone calls, targeted campaigns, scheduling appointments in the electronicbrmedical record system, following up to determine if scheduled appointments werebrkept, and monitoring gaps in care outlined by health plans. This positionbrcommunicates accurate electronic messages to appropriate care team member asbrneeded to address patients calling with questions for their care team. /p p /p p

Duties:

  • Inbound telephone call from multiple sitesbrthrough call queue. /p p
  • Accurate transmittal of medical messages to carebrteam. /p p
  • Scheduling of appropriate patient appointments. /p p
  • Reviewing of historical appointments tobrdetermine appropriate appointment types and duration. /p p
  • Timely follow up on missed call opportunities. /p p
  • Review of after-hours call registry to ensurebrpatient appropriately scheduled. /p p
  • Contact patients that have been linked to clinicbrthrough their insurance plan or benefit plan but have never been to clinic tobrschedule them an appointment. /p p
  • Answer patient’s questions regarding whatbrservices clinic offers to patients and clinic’s availability. /p p
  • Scheduling patients for wellness visits andbraddressing gaps in care. /p p
  • Insurance payer outreach campaign calling asbrdirected. /p p
  • Followbrup on no show patients, past due to be seen patients. /p p
  • Communicate information to program leadershipbrregarding patients that prove difficult to reach or who elect not to setbrappointments. /p p
  • Assist with documenting and tracking PopulationbrHealth campaign calls and volumes. /p p
  • Daily reporting as directed on call volumes,brequipment failures, patient complaints. /p p
  • Referring of patients calling with SocialbrDeterminants of Health. /p p
  • Other tasks and duties ass assigned bybrmanagement or leadership.

Required Qualifications:

  • High School Graduate /p p
  • 1 year experience in a health care or medicalbroffice setting /p p
  • Experience working with a diverse population /p p
  • Excellent verbal and written communication skills /p p
  • General computer literacy experience withbrMicrosoft Windows, Microsoft Office /p p
  • Knowledge of basic medical terminology /p p
  • General knowledge of medical insurance industrybrstandards /p p /p p
Preferred Qualifications:

  • 1 year experience working with an electronicbrmedical record /p p
  • 1 year experience working in health care qualitybrassessment, measurement and/or analysis i.e. HEDIS measures /p p
