The clinic operates in partnership with various healthcare professionals, organizations, and volunteers to address the unmet healthcare needs of the community. Role Description This is a full-time on-site role as a Bilingual Patient Navigator/Enrollment Specialist at the Community Free Clinic of Cabarrus County located in Charlotte Metro. The Bilingual Patient Navigator/Enrollment Specialist will provide enrollment services for individuals applying for the HealthNet Cabarrus Pink Card Program. The employee will serve as the first point of contact to explain the HealthNet Cabarrus program, manage applications and supplemental documents, and assess program eligibility. The new employee will screen for social insecurities that patients may face and connect them to community resources that address those concerns. They will help patients better understand the HealthNet Cabarrus network and facilitate specialist referrals for patients assigned to partner agencies, ensuring active communication with both patients and their referring providers. The work week is Monday – Thursday 8 am – 5 pm with four additional hours for outreach to be conducted on weeknights, Fridays during the day, or weekends for a total of 40 hours per week.

General Description:

Essential Functions Assess eligibility of applicants for the HealthNet Pink Card Program. Process applications for HealthNet Cabarrus, MedAssist, and other financial assistance programs, working with patients to ensure all information is entered accurately Complete the annual renewal process for current HealthNet participants in a timely manner. Organize and maintain paperwork and chain of custody for documents. Collaborate with contacts at partner agencies for programming and patient referrals Educate patients about HealthNet Cabarrus and the healthcare system at large Understand patients’ social determinants of health and offer resources for support Build new relationships within the Charlotte metro area to expand the resources, programming, and services available to our patients Maintain relationship with current community partners to provide the necessary bridge for patients seeking gap services. Host/Attend local outreach events to connect with potential patients and partners Work with department to create an annual outreach plan for the enrollment team Complete all work within required timeframes to meet goals and strategic plans Other duties as assigned

Education/Experience:

Must be fluent in Spanish and English 1-3 years social service or nonprofit work desired Knowledge of healthcare terms preferred and willingness to learn College degree desired but not required

Strong communication and interpersonal skills Comfortable with technology and a variety of computer applications, and willingness to learn new software Maintain clear communication with coworkers, partners, and patients Exercise good judgment in appraising situations and making decisions Show empathy, compassion, and no judgment of patient circumstances Support the Clinic’s mission of improving access to healthcare and removing barriers to healthcare Maintain effective working relationships Problem solve and think critically when serving patients, working partners, and supporting the Clinic’s function
