Enrollment Coordinator What is Carolina SeniorCare? Carolina SeniorCare is a P.A.C.E. (Program of All-inclusive Care for the Elderly). We are the best kept secret in Davidson, Davie, Rowan and Iredell which are the counties we serve. P.A.C.E. offers a wide variety of services to fulfill this mission such as medical services to medications, from nutritious meals to transportation – all personalized to meet the participant’s specific needs!

QUALIFICATIONS

1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. 2. Must have a valid driver’s license, proof of insurance and have means of transportation. Must be able to meet Carolina SeniorCare’s auto insurance carrier’s standards for coverage. 3.

Education:

 Associates degree required. Bachelor’s degree preferred. Current Cardio Pulmonary Resuscitation (CPR) certification required. 4. 

Experience:

 Two to three years experience in Health and Human Services in case management, benefits, administration, or social work in a health care environment.
Computer proficiency in word processing and spreadsheet applications. Minimum of one year working with the frail or elderly. 5.

Skills and Knowledge:

Ability to effectively present PACE philosophy and model to the community and to prospective participants.
Working knowledge of physical, psychosocial, behavioral and family needs of the elderly.
Working knowledge of community health and social service delivery systems and aging networks.
Excellent written and verbal communication skills.
Able to assess potential participant’s financial status.
General knowledge of financing mechanisms, such as Medicaid and Medicare.
Computer proficiency in word processing and spreadsheet application.
Able to work independently and function with minimal supervision.

ESSENTIAL FUNCTIONS

 : 1. Receives and provides information to referrals to the PACE Program; refers individuals to other appropriate resources as indicated. 2. Assesses potential PACE participants in homes, nursing homes, hospitals or other settings as part of the intake process. 3. Maintains current, written assessment, and intake records inclusive of potential participant’s expressed wishes and the referral services provided, as appropriate. 4. Assists potential participants in managing their needs while undergoing intake and assessment process, including referrals to doctors and other community agencies. 5. Participates in Interdisciplinary Team meetings when potential enrollees are assessed for enrollment into the program for purposes of providing and clarifying information obtained during the intake assessment. 6. Provides alternative community resources to applicants who are not accepted into the
PACE program. 7. Assists with financial eligibility process for potential PACE participants. 8. Reviews Medicaid eligibility, monitors time frame for recertification and facilitates
Medicaid applications for certification and recertification. 9. Collects data and prepares reports in accordance with the State PACE Administering agency and CMS requirements. 10. Assists in evaluating the intake and assessment process, identifying problem areas accordingly and making changes as recommended. 11. Contacts identified key referral sources regularly to discuss possible program candidates. s responsibility with other team members in the implementation of the PACE philosophy and achievement of program goals. 12.
