Position Summary:
We are seeking a dynamic, compassionate, and highly organized Hospital Liaison to serve as the key point of contact between hospitals, referral sources, patients, and our facility. This role is essential in promoting our services, streamlining the admissions process, and ensuring a seamless transition for patients from the hospital setting to our care environment.
Key Responsibilities:
Build and maintain strong relationships with hospital discharge planners, case managers, physicians, and social workers. Promote facility services through hospital visits, presentations, and follow-up with potential referral sources. Facilitate smooth and timely patient admissions by coordinating with families, hospitals, and internal departments. Conduct pre-admission screenings and assessments to ensure clinical and financial eligibility. Maintain accurate documentation and records in accordance with regulatory and company standards. Provide exceptional customer service to patients and families, addressing concerns and providing support during the transition. Monitor and report referral and admission trends to the leadership team. Collaborate with the interdisciplinary team to support census growth and maintain a positive reputation in the healthcare community.
Qualifications:
Associate’s or Bachelor’s degree in healthcare, marketing, or related field (preferred). Minimum 2 years of experience in healthcare admissions, marketing, or hospital liaison work. Knowledge of Medicare/Medicaid, insurance, and the post-acute care continuum. Strong interpersonal, communication, and organizational skills. Ability to work independently and manage multiple priorities. Reliable transportation and willingness to travel locally to hospitals and other healthcare facilities.
Preferred Skills:
Familiarity with CRM or referral tracking systems. Bilingual abilities are a plus. Background in skilled nursing, rehab, or long-term care preferred.
