Job Description
Description The Family Support Specialist is responsible to serve as a liaison for families and establish relationships with infants and their families in Early Intervention and/or Preschool. They will schedule, conduct meetings, make referrals and ensure services are provided as per the IFSP. Core Responsibilities + Serve as a liaison and primary contact for families affiliated with the Early Intervention and Preschool. + Provide ongoing service coordination across Oneida, Herkimer and Madison County. + Manage student educational records for quality assurance focused on accuracy, timelines and content. + Participate in meetings with staff, parents, and community members to ensure the child and family’s needs are met. + Perform administrative support by attending meetings, participating on committees and other activities as required. + Ensures all services are established and maintained for the families. + Assures supports are in place inclusive of service provider and family contacts. + Collaborates with all service providers and establishes team communication plan. + Advocates for additional services and linkages as appropriate. + Provides written documentation of services. + Participates in NY State regulatory training as required. Qualifications + 4-year Degree in Human Services or related field. + At least 2-years’ experience working with Early Childhood populations. + Must have or obtain the NYS Early Intervention Coordination Certification within the first 3-months in this position. + Must complete 40 hours of specialized/designated training within the first year in this role per regulatory requirements. + Travel is required.
